I hereby give my consent for my child to participate in the Tennessee Golf Foundation (TGF) program. I also consent to have my child receive medical attention in an emergency situation, in the event that it is impossible to reach me personally.

I, the parent/legal guardian of the above-named youth, give approval for participation in the TGF sponsored activities during the calendar year 2024. I understand and assume the risk and danger incidental to the game of golf, including, but not limited to, the risk of my child being hit by an errant or misdirected golf shot and the risk of my child causing injury to another person or damage to the property of another. I assume all risks of injury whatsoever and agree to hold harmless: Tennessee, Tennessee Golf Foundation, Tennessee Junior Golf, NLLC, Tennessee Section PGA of America, Inc., Golf House Tennessee, Metropolitan Board of Parks and Recreation (Metro Parks), Chattanooga Junior Golfers’ Development Program, and the agents and employees thereof, from any and all liability from such causes that might occur while my child is a participant of from claim(s) of any nature arising from any activity, including transportation, connected with the TGF facility or program. This holds harmless agreement includes, but is not limited to, any claim due to injury resulting from the negligence of the TGF, its employees, agents, LPGA and PGA Professionals, participating agencies, and volunteers. I consent to the TGF communicating information regarding my child’s participation via the internet. This consent form is valid for all activities of TGF for the calendar year of 2024.

Participants, parents, or guardians understand that publicity photographs may be taken during the event, and their image/photographic likeness may appear in photographs taken during a variety of events. By registering for TGF programming you hereby give permission that photos containing your image/likeness may be used for publicity or general information purposes, website or within other publicity which may be seen by the general public. TGF will not seek any further permission nor provide any notification before using such photos.
